I understand that tattooing involves the insertion of pigments into the skin using a tattoo machine or hand tools.

  3. Health Considerations: I confirm that I am in good health and that I have disclosed any relevant medical conditions, allergies, or sensitivities to the tattoo artist. I understand that tattooing may not be advisable for individuals with certain medical conditions, compromised immune systems, or skin conditions.

  4. Risks and Complications: I understand that there are risks associated with the tattooing process. These may include, but are not limited to, pain, bleeding, infection, allergic reactions, scarring, and dissatisfaction with the final result. I acknowledge that I have been informed of these risks and I accept them.

  5. Consent to Tattoo: I hereby give my consent to receive the tattoo as described above. I understand that the tattoo artist will make every effort to achieve the desired design and placement, but variations may occur due to factors such as skin texture, healing process, and artistic interpretation.

  6. Photography Release: I grant permission to [Tattoo Studio Name] to take photographs or videos of the tattoo process for documentation, marketing, or promotional purposes. I understand that my identity will be kept confidential, and my tattoo may be cropped or edited to maintain anonymity if necessary.

  7. Care and Aftercare: I understand that proper aftercare is essential for the healing and longevity of the tattoo. I have been provided with aftercare instructions and I agree to follow them diligently. I will seek medical advice if any complications arise during the healing process.

Release of Liability: I release [Tattoo Studio Name], its artists, employees, and representatives from any liability for any injury, infection, allergic reaction, dissatisfaction, or other complications that may arise as a result of the tattooing process, except in cases of negligence or intentional misconduct.

    • Pregnancy or breastfeeding: Tattooing during pregnancy or while breastfeeding is generally not recommended.

    • Impaired immune system: Individuals with compromised immune systems, autoimmune disorders, or undergoing immunosuppressive treatments may have an increased risk of complications.

    • Allergies or sensitivities: Any known allergies or sensitivities to tattoo ink, latex, or other tattoo-related products should be disclosed.

    • Skin conditions: Active skin conditions like eczema, psoriasis, or dermatitis in the tattoo area may increase the risk of adverse reactions or hinder the healing process.

    • Bloodborne diseases: If the client has a bloodborne disease such as HIV, hepatitis, or any other infectious condition, the tattoo artist needs to take extra precautions or may decline to perform the procedure.
