PERMANENT MAKEUP CONSENT FORM

This form provides information to assist in making an informed decision of whether or not to undergo a permanent cosmetics application. If you have questions, please don‘t hesitate to ask. Although permanent cosmetic tattooing is effective in most cases, there are biological factors specific to any individual client that are beyond our artist’s control. Every effort will be made to ensure optimal results, however no guarantee can be made that a specific look or outcome will result from the procedure.

During the permanent cosmetics procedure, pigment is deposited into the dermal layer of the skin and is a form of tattooing. All instruments that enter the skin or come in contact with body fluids are sealed and sterilized before use and disposed of after use. Cross-contamination and bloodborne pathogen guidelines are strictly adhered to.

 

Generally, the results are excellent. However, a perfect result is not a realistic expectation after an initial application.  It is usual to expect a touch-up after the healing is completed. We recommend two sessions to achieve optimal desired results. However, in certain cases, subsequent sessions may be needed in which case any corresponding fees will apply. Initially, the color will appear much more vibrant and/or darker compared to the end result.  Usually, within 5-7 days the color will shrink about 25% in size and the color will soften about 40-50%. The pigment will fade naturally over time and will likely need to be touched up through the years.

POSSIBLE RISKS, HAZARDS, OR COMPLICATIONS

 

• Pain: There can be pain even after the topical anesthetic has been used. Anesthetics work better on some people than others.

• Infection: Infection is very unusual, but can occur. The areas treated must be kept clean and only freshly cleaned hands should touch the areas.  See “After Care” sheet for instructions on care. 

• Uneven Pigmentation: This can result from poor healing, infection, bleeding or many other causes.  The follow up appointment can help correct any uneven appearance.

• Allergic Reaction:  If there is any concern for an allergic reaction to pigments, a patch test can be done. However, if a allergic reaction to occur, it may not show up for a long period of time, making the patch test inconclusive.

• Asymmetry: Every effort will be made to avoid asymmetry however, anatomical facial features may not be symmetrical. Adjustments may be needed during the follow up session to correct any unevenness.   

• Excessive Swelling: Clients with sensitive skin are more prone to swelling. This should dissipate within 24 hours.

• Acne prone: If acne is present on the forehead area, the brows may not heal correctly. If acne is a consistent issue, please consider having permanent micro pigmentation done is not in your best interest.

• Anesthesia: Topical anesthetics are used to numb the area to be tattooed.  Lidocaine, Prilocaine, Benzocaine, Tetracaine and Epinephrine in a cream or gel form are typically used. If you are allergic to any of these please inform us now. 

• MRI: Because pigments used in permanent cosmetic procedures contain inert oxides, a low level magnet may be required if you need to be scanned by an MRI machine. You must inform your technician of any tattoos or permanent cosmetics.

 

THE FIRST SEVEN DAYS

 

The first week following your cosmetic tattoo treatment is crucial in ensuring effective healing and a great result. Your practitioner has done all they can to give you beautiful permanent eyebrows; now it’s up to you to finish the task. During the first seven days, we recommend:

  • Using your ointment as instructed: Your practitioner may have given you a healing balm to use. This should be gently applied two to three times daily, as instructed, using clean hands and a clean cotton bud for each application. Remove any excess with a clean tissue.; less is more and your skin needs air to breathe and heal. With some Eyebrow tattoo techniques we even advise dry healing, with minimal or no use of any ointment at all. Your practitioner will advise you personally about all this during your consultation.

  • Resist the urge to scratch your skin: Some itching is normal; it’s a sign that your skin is starting to heal itself. Do not pick at or scratch the treated area as this could cause scarring, infection and a poor colour result. Your eyebrow tattoo may flake a little; this is also perfectly normal and should be left to heal naturally.

  • Avoid makeup and products: In the area where you have had your eyebrow tattooing done, you should not apply anything other than the supplied healing balm. Any moisturisers, makeup, oils or lotions could cause infection or a reaction in your skin, which will be detrimental to your end result.

  • Don’t wash your face: This might sound a bit gross but getting water on the treated area can cause problems with the semi permanent tattoo. Instead of splashing water all over your face, use a cleanser to clean everywhere but the tattoo itself. If you do get your eyebrow tattoo wet, pat it dry gently with a clean tissue.

  • Pin back your fringe: If you often have hair in your face or a fringe, take care to clip it back for the first week at least. Hair getting into your tattooed eyebrows can cause infection which will disturb the way the colour sets.

  • Avoid sun and sunbeds: Large sunglasses or a brimmed hat can offer great protection for your eyebrow tattoos from the sun. Don’t go on sunbeds or take part in any activity that could cause excessive sweating such as sports or saunas.

Taking care of your tattoo at this early stage can mean the difference between a beautiful, long lasting cosmetic tattoo or a disappointing result. Remember, your colour will inevitably be darker now than the end result, so don’t assume that the colour is wrong; give it time and it will soften to the perfect shade you wanted.

THE FIRST FEW WEEKS

Most eyebrow tattoos will have healed well by the time two weeks are up. During these two weeks, you should take care to avoid sunbeds, sunshine, swimming pools, salt water and direct shower spray. You don’t need to be quite so careful about washing your face, as long as the water is cool, and can finally get your fringe back down without worrying about infection.You should have been given a follow up appointment by your practitioner for four to eight weeks after your initial treatment. This is a great opportunity to evaluate the colour and shape now that things have settled down. Your semi permanent makeup practitioner will go over your brows, re-applying more pigment and addressing any issues of patchiness or uneven shape.Just as when you had your first makeup tattoo done, you should take good care of your eyebrows for the first seven days following your touch up. Even after this, do avoid rubbing or scratching your eyebrows for several weeks, as this could cause premature ink removal and result in your tattoo fading too fast.

LONGER TERM AFTERCARE FOR YOUR PERMANENT EYEBROWS

 

You’ve invested good money in these gorgeous new eyebrows, so naturally you’ll want to get the best value from your investment. Helping your eyebrow tattoos to stay sharp and nicely coloured means you’ll need to take care of them long term too, although really this care is fairly minimal. We recommend:

  • Always using sunscreen, as sunlight will fade your semi permanent eyebrow tattoos

  • Avoid using anti-aging creams in the area, as they can affect your tattoos

  • Avoid certain skin treatments in the area, such as chemical peels, facial scrubs, laser rejuvenation, microdermabrasion and others

  • If you plan a skin treatment, tell your beautician about your permanent eyebrow tattoo

  • If you need an MRI scan or other medical procedure, inform your doctor about your semi permanent eyebrows

Although all these Permanent makeup aftercare instructions may seem a little daunting at first, most of it is simply common sense. Your practitioner is always at the end of the phone for support, whether it’s been a month or a year since you had your treatment.There’s no guarantee as to how long semi permanent brows will last, but with a little care and consideration, you’ll ensure you achieve a great result for as long as possible.

What is Normal

 

• Swelling, itching, scabbing, light bruising and dry tightness. Ice packs are a nice relief for swelling and bruising. Aftercare balm is nice for scabbing and tightness. 

• Too dark and slightly uneven appearance. After 3-7 days the darkness will fade and once swelling dissipates unevenness usually disappears. The follow-up touch up is highly recommended as any concerns can be addressed.

• Color change or color loss. As the procedure area heals the color will lighten and sometimes seem to disappear. This is normal. The procedure area has to be completely healed before we can address any concerns. This takes at least four weeks. 

• Needing a touch up months or years later. A followup touch up is recommended 6-9 weeks after the initial appointment. Thereafter maintenance may be needed every 12-18 months to keep the shape symmetrical and the color refreshed. Failure to follow after care instructions may result in infections, pigment loss or discoloration. 

 

STATEMENT OF CONSENT AND RECITALS:

 

I certify that I am over the age of 18, I am not under the influence of drugs or alcohol, I am not pregnant or nursing, and I consent to receiving the indicated micropigmentation or permanent cosmetic procedure.

 

The general nature of cosmetic tattooing as well as the specific procedure to be performed has been explained to me. I have been informed of the nature, risks, and possible complications and consequences of permanent skin pigmentation. I understand the permanent skin pigmentation procedure carries with it known and unknown complications and consequences associated with this type of cosmetic procedure, including but not limited to: infection, scarring, inconsistent color, and spreading, fanning or fading of pigments.

 

I understand the actual color of the pigment may be modified slightly, due to the tone and color of my skin. I fully understand this is a tattoo process and therefore not an exact science, but an art. I request the permanent skin pigmentation procedure/s, and accept the permanence of the procedure as well as the possible complications and consequences of the said procedure/s. 

 

There is a possibility of an allergic reaction to pigments. A patch test is advisable however it does not ensure a client will not have an allergic reaction. If waived, I release the technician from liability if I develop an allergic reaction to the pigment. I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my permanent cosmetics.

 

I acknowledge some of these potential adverse changes may not be correctable. I have received pre- and post-procedure instructions and I will strictly adhere to such instructions. I understand that my failure to do so may jeopardize my chances for a successful procedure. If I am on any medication for depression or any other mood altering prescription, I will advise my technician. If I have ever had cold sores, I will consult with and strictly follow my doctor's instructions before contemplating any permanent cosmetic procedure around my lips.

I understand that before and after photographs of the said procedure/s are conditions of such procedure/s. I certify I have read and initialed the above paragraphs and have had explained to my understanding this consent and procedure permit. I accept full responsibility for the decision to have this cosmetic tattoo work done and understand that there is a no refund policy. I understand that the cost of touch-up’s are not included in the procedure and the cost of touch up’s differs as time lapses from the original date procedure was dine.

Credit Card Guarantee: To secure your appointment and guarantee that a deposit is paid, we may require your credit card information when booking an appointment. This information will be securely stored and will only be charged in the following circumstances:

  1. Cancellation Policy: We understand that circumstances may change, and you may need to reschedule or cancel your appointment. To avoid any charges, we kindly ask for at least 48 hours' notice for cancellations or rescheduling. If you provide us with sufficient notice, no charges will be applied to your credit card.

  2. No-Show Policy: A "no-show" is when an appointment is missed without any prior notice or cancellation. In the event of a no-show, your credit card on file may be charged a fee as outlined in our pricing policy. We reserve the right to charge your credit card for the full cost of the missed service.
