It is very important that you indicate all that apply to you so that the technician can properly advise you regarding whether the service is right for you and what you can expect in terms of results and post-operative care.

  • I understand that if I am late to my appointment there is no guarantee that my appointment can be accommodated.

  • I accept that if I cancel any appointment without providing 24 hours notice per the 12-hour cancellation policy I won’t be charged for the missed appointment but I will be required to pay 20% in advance for all future appointments, which amount is non-refundable even if I cancel within the 24-hour cancellation policy.

  • I understand and agree to the aftercare instructions below.

  • I understand and agree that failure to adhere to these aftercare instructions will interfere with optimal results and will likely result in premature loss of eyelash extensions, damage to the eyelash extensions or cause a lash lift to fall.

* IMPORTANT * All Permanent Makeup, Lashes, and Eyebrows treatments are exclusively administered by LH Beauty Salon, operating within the premises of Mejor Vida Medical Spa. LH Beauty Salon operates as an independent entity and maintains no affiliation with Mejor Vida. Please be advised that when scheduling any treatment with LH Beauty Salon, it is understood that Mejor Vida Medical Spa bears no responsibility or liability for any issues or conflicts that may arise during or after the treatment. By affixing your signature to this Consent form, you acknowledge and agree to absolve Mejor Vida of any disputes, damages, lawsuits, complaints, or other issues that may arise in connection with the services rendered by LH Beauty Salon. *Risks & Complications*

 

I understand and accept the following risks exist:

• eye irritation including redness

• eye pain

• eye itching

• eye infection

• in rare cases, loss of vision can occur

*Term of Consent Form and Release*

 

This agreement will remain in effect for this procedure and all future procedures conducted by any stylist at the studio. I release my stylist from all liability associated with this procedure.By signing below, I verify that I am over 18 years of age and have read, understand and agree to everything contained in this consent form.

I give permission to take before and after photos of my eyes/ face which may be used for any type of marketing purposes (website, brochures, business cards, salon or class, etc).

Credit Card Guarantee and No-Show Policy

We kindly request your cooperation in adhering to this policy for all appointments.

 

Credit Card Guarantee: To secure your appointment and guarantee that a deposit is paid, we may require your credit card information when booking an appointment. This information will be securely stored and will only be charged in the following circumstances:

  1. Cancellation Policy: We understand that circumstances may change, and you may need to reschedule or cancel your appointment. To avoid any charges, we kindly ask for at least 48 hours' notice for cancellations or rescheduling. If you provide us with sufficient notice, no charges will be applied to your credit card.

  2. No-Show Policy: A "no-show" is when an appointment is missed without any prior notice or cancellation. In the event of a no-show, your credit card on file may be charged a fee as outlined in our pricing policy. We reserve the right to charge your credit card for the full cost of the missed service.

When clients miss appointments without notice or repeatedly cancel at the last minute, it can disrupt our schedule and affect our ability to serve other clients.

How to Provide Your Credit Card Information: When booking an appointment, our booking site will guide you through the process of securely providing your credit card information. You will also receive a confirmation email with the details of your appointment and the policies for your reference.
