EYEBROW WAX TINT AND BROW LAMINATION CONSENT FORM

Eyebrow correction is an eyebrow hair removal process using soy based surgical wax. There will be some tweezing as well. A topical antiseptic anti-bacterial cream will be applied to avoid any bacterial infection in occurrence.

 

Tinting is a brow hair dye applied to the hair and skin and contains formaldehyde in thedeveloper. Brow Lamination is a process of reconstructing the brow hairs to keep them in a desired shape. The tint is an optional service.

 

Brow Lamination lasts approximately 6-8 weeks; this will differ based on the type of hair and skin type.

I Understand the Following:

• During the treatment, despite all the precautionary measures made by the technician, injury is possible. I will not hold the technician performing this service on me responsible in any issues that may arise as a result of having the Brow Lamination procedure performed on me.

 

• Despite the application of the most advanced and top ingredients, an allergic reaction is possible.

 

• It is my responsibility to advise the technician of any concerns I may have before the Brow Lamination procedure. Even though I may have written it down on this form.

 

• The minimum or maximum duration of the Brow Lamination cannot be determined with certainty.

 

• The technician performing the Brow Lamination will not be held liable for and damages caused to me or my eyebrows by any reason, including allergic reaction, to previous procedures such as previous henna/tint on the brow hair, skin sensitivity, and failure to follow the Brow Lamination after care instructions.

*Brow Lamination is NOT recommended if you have the following: *

 

• Psoriasis/severe Eczema

• Recent Eye Surgery

• Alopecia

• Recent Microblading (must be healed over 8 weeks)

• Sunburn

• Super Sensitive Skin

• Blood Thinners

• Pink Eye

• Scar Tissue in the Treatment Area

• Pregnant/Breastfeeding

• Retinol, Accutane, AHA, BHA, etc

• Current or Recent Chemotherapy Treatments (Doctors approval required in either case)

 

By signing below, I confirm to the best of my knowledge that the information I have provided is accurate and complete. I have not withheld any information that may be relevant to treatment and/or the result. I further agree that I will not hold the technician responsible should there be any unfavourable results.

By signing below, I consent to photos taken before and after treatment and may be used on social media, however client privacy is important to us and any photo used will be cropped appropriately if required.

Credit Card Guarantee: To secure your appointment and guarantee that a deposit is paid, we may require your credit card information when booking an appointment. This information will be securely stored and will only be charged in the following circumstances:

  1. Cancellation Policy: We understand that circumstances may change, and you may need to reschedule or cancel your appointment. To avoid any charges, we kindly ask for at least 48 hours' notice for cancellations or rescheduling. If you provide us with sufficient notice, no charges will be applied to your credit card.

  2. No-Show Policy: A "no-show" is when an appointment is missed without any prior notice or cancellation. In the event of a no-show, your credit card on file may be charged a fee as outlined in our pricing policy. We reserve the right to charge your credit card for the full cost of the missed service.
